Post-Secondary Transition Planning
At SuccessAbility, we are proud to offer a comprehensive Youth Transition Program designed to help students with disabilities such as Autism Spectrum Disorder, ADHD, learning disabilities, cerebral palsy, concussions, and physical disabilities successfully navigate the transition to post-secondary education. Our program ensures that each student is supported in every aspect of their transition—from academics to independent living—helping them develop the necessary skills for sustained success. We integrate a neurodivergent-affirming approach throughout the program, recognizing and celebrating the unique strengths and needs of every student.

At SuccessAbility, we recognize and embrace the neurodivergent nature of many of our students. Our neurodivergent-affirming approach integrates the belief that neurodiversity is a natural and valuable form of human variation. Navigating Services and Support
Transitioning to post-secondary education can be overwhelming, especially when students are faced with unfamiliar systems and services. Many students struggle to communicate with university administrators or advocate for themselves in the often-complex world of higher education. At SuccessAbility, we provide guidance to ensure that students are prepared to:
Disability Services Offices & Accommodations: We assist students in understanding and accessing the accommodations they are entitled to, connecting them with the disability services offices on campus. Whether they need additional time for exams, assistive technologies, or support for note-taking, we ensure that the right services are recommended to help them succeed.
Federal and Provincial Disability Grant Funding: Our team helps students identify and apply for available disability grants and funding programs, making it easier for them to secure financial assistance for tuition, assistive technologies, and living expenses.
Tutoring & Learning Strategies: We connect students with tutors and learning strategists, providing tools to help them manage coursework, study effectively, and organize their academic responsibilities.
Course & Degree Planning: We assist students in choosing the right courses, navigating degree requirements, and planning a personalized academic path that aligns with their goals and interests.
Residence Planning: We help students understand residence options, including accessible housing and residence support services, to ensure they have a comfortable and supportive living environment during their time in post-secondary education.
Building Foundational Skills for Independence
In addition to academic support, our program focuses on equipping students with the skills they need for successful student life. We recognize that many students with disabilities face challenges in planning and engaging in sustainable strategies for activities of daily living. Many students struggle with organizing and managing the skills required for self-sufficiency, which can impact their ability to thrive in post-secondary settings. Our program addresses these challenges in a collaborative manner to ensure students develop and refine these critical skills, individualized to their unique strengths and needs, in advance of their transition:
Activities of Daily Life (ADLs): We provide training in essential life skills, such as grocery shopping, meal planning, cooking, cleaning, personal hygiene, and organization. This training is designed to help students develop sustainable strategies for managing daily routines, which is essential for independent living and overall well-being. We emphasize the importance of continuous iteration on these strategies, helping students build a solid foundation of skills that they can adapt and refine as their needs evolve.
Social Connections & Leisure: We support students in building meaningful social relationships by encouraging participation in social activities and fostering a sense of community. We also focus on the importance of leisure and self-care activities, promoting balance and mental health as essential components of a successful and fulfilling life.
Mental Well-Being & Stress Management: Mental health is a priority in our program. We teach students effective strategies for managing stress and maintaining emotional well-being. This includes techniques for mindfulness, relaxation, and emotional regulation, which are essential for coping with the demands of academics and personal life.
Self-Management & Time Management Skills: We help students develop the skills necessary for managing their academic workload alongside personal responsibilities. This includes time management strategies, goal setting, and creating routines that enable students to stay organized and on track. We emphasize the importance of ongoing self-reflection and the ability to adapt strategies to maintain academic success and overall well-being.
